Appointments and Communication
Patients are responsible for providing accurate health information, completing required forms, attending scheduled appointments, and communicating any changes in health status, medications, symptoms, or side effects.
Telehealth services may not be appropriate for every condition. Emergency symptoms should be directed to 911 or the nearest emergency department.
Payments, Memberships, and Cancellations

Missed appointments or late cancellations may be subject to a fee.

Revive Nation may coordinate with third-party labs, pharmacies, payment processors, scheduling systems, and patient portal platforms. Revive Nation is not responsible for delays, errors, availability issues, pricing changes, or service interruptions caused by third-party vendors.
Medications are prescribed only when medically appropriate and at the discretion of the licensed provider.
